Bell you may send an e-mail to rebate@unical-usa.com
with the following information. Your first and last name as written
on the rebate form. Your zip code, daytime phone number and the
date of purchase. Or you may contact us by phone at 1-800/423-4653
x 151. We will research your entry and reply back by e-mail
or phone.
